Courtland Hospice - What We Offer

We know that medical needs are unique to an individual. Our team will work with patient and family to create a care plan that is tailored to their specific needs and situation. 

We administer prescribed medications including comfort and pain medications.

Our spiritual support team provides spiritual care to patient and family throughout the entire end-of-life journey

Our respite services team assist in alleviating caregiver burden by managing patient care at a facility

We connect local volunteers to patients and families through our competent team of volunteer coordinators

Our team of CNAs and PCTs assist patients with their activities of daily living including bathing, feeding, personal grooming in the comfort of their home

The bereavement team will be available to assist in the grief process post-passing for family and friends of our beloved patients. Resources such as support groups, individual meetings, and written literature are provided during this process. 

Through our partners, we provide Durable Medical Equipment (DME) to support the needs of the patient.

For patients with uncontrolled symptoms, we provide round-the-clock care and support in their home until the symptoms are brought under control
